Chicago Medical ID Program

The Chicago Medical ID program offers a free medical identification card to Chicago residents of all ages who want their medical information and vital statistics readily available. Chicago Medical ID Application (PDF)

The front of the identification card includes: Name, address, date of birth, emergency contact, phone number, and photo.

The reverse side of the card includes: Primary Care Physician name and phone number, medical conditions (if any), blood pressure status, and any medical allergies.

Chicago residents can apply for a Medical ID card at City Hall, 121 N. LaSalle St., Room 107 or at both of our satellite office at 5674 S. Archer Ave., Unit A and 5630 W. Gale St. All office locations provide this service Monday through Friday, 8 am to 5 pm. You must present a valid form of identification at the time of service.

City Clerk del Valle's mobile Community Services Division travels to community events throughout the City providing free KIDS and Medical ID programs. To schedule an on-site program for groups of 50 or more participants, contact the Community Services Division of the Office of the City Clerk
