Overview
The Chicago Medical ID program offers a free medical identification card to Chicago residents of all ages who want their medical information and vital statistics readily available. This service is free to Chicago residents.
How to Get Your Card
The process is pretty simple and takes only a few minutes:
- Cardholder completes and signs the Personal/Medical information form.
- Clerk staff take information from the form and a digital photo (taken onsite or sent to our office) to create the card using a specialized system and provide the card to the resident.

This transaction can occur via any of the following means:
- In person at two of our Office of the City Clerk locations
- In person at any of our dozens of community events throughout the year. (Forms will be available at the event.)
- Via mail: Send the completed form and a photo to The Office of the City Clerk, Community Services, 121 N. LaSalle St., Rm. 107 Lower Level, Chicago, IL 60602
- Via email: Email the completed form and a digital photo or JPEG to Louisa.Keefe@CityofChicago.org
Storing and Sharing the Information
The Clerk’s Office values our residents’ privacy and the information and pictures provided will be not be shared and will be stored securely in the Clerk’s office.
