Senior Citizen Medical I.D. Program
The Chicago Senior Citizen Medical I.D. Program is a service provided by the Office of the City Clerk. As the official record keeper of city government, the clerk's office has setup this high-tech system to keep our seniors safe. The program entails the senior filling out the appropriate information on an application along with taking a digital photograph. The senior's photograph and information will be securely stored by the city clerk's office. The information will be readily available to law enforcement agencies and medical professionals for the purpose of identifying the named person if they become incoherent or unconscious. In order for a senior to process the information for the medical I.D. card, we ask that another form of identification be shown at the time of application. The I.D. card will include the following information: the individual's picture, name, address, city/state, birth date, emergency contact, contact number, medical conditions, other conditions, medical allergies, blood pressure, doctor, doctor's number and insurance company. The medical I.D. card will then be mailed to the individual's place of residence 2-3 weeks following the appointment. This service is free to all senior citizens ages 60 and over.
